Make sure you incude this in any newspaper ads or flyers (flyers are available for download from your ads web page for free.) Another great idea is to write the url address on your for sale sign.
The more complete the form the better response you'll have. Remember to write a detailed description of your home in the short and full description blocks. You may want to include things that you enjoyed while living in your home.
